7 Day Nursing Service in South Dublin Area

This service is piloted with SJH and AMNCH and covers the Local Health Areas of Dublin South City, Dublin South West, Dublin West, Dublin South East & Dublin South.  The primary aim of this service is to free up beds in acute hospitals by:
1. Faciltating early discharge of clients home from acute hospital setting and
2. Preventing unnecessary admissions to A/E's by dispatching nursing intervention to clients in their own homes.

Referrals 9.30am to 9.30pm

Interventions include:
IV antibiotic administration
PICC/HICC/PORT care incl chemo pump disconnections
Subcutaneous injections
COPD early discharge assessments
INR monitoring with heparin bridging
Suprapubic re catheterisation
Evening support calls

Fax referral form directly to  01-4987132 to include clients name, dob, address, contact phone number and reason for referral.
All referrals will be confirmed with referrer
